Affordable Care and Insurance

We accept a growing list of insurance plans. We also offer affordable cash pay appointments to those with a need.

 

Many insurance companies will allow you to submit your cost for out-of-network reimbursement and we can do that for you if you'd like. We also offer a sliding scale and payment plan to those in need. This keeps our overhead low and passes the savings to you.

Many patients will find our prices the same they pay out of pocket, and we will probably fit you in sooner.

Note!: First Responders and Medical Personnel get special rates for 1 year for all that you've been doing to support us during COVID.

    • Tricare - Try as we might it's still about a year out - but we support our service members so ask about discounted cash rates or out of network options.

* Please note that insurance companies have many sub-plans and networks. If you want to be certain that your plan is covered, we can check for you, please contact us with your name, address, plan ID, and group number.

We offer longer and more affordable appointments than many of our peers in the area.

Our fees are subject to change. (Current patients will be notified in advance of any change):

  • Free 10-minute meet and greet – Free

  • 90-minute new patient intake – Based on your insurance. If you'll be paying out of pocket, contact us for slide scale rates or payment plans.

  • 30-minute follow-up – Based on your insurance. If you'll be paying out of pocket, contact us for slide scale rates or payment plans.

  • Gender-affirming surgery and medical letters are FREE to current recurring patients. We also offer a limited number of monthly free appointments for non-patients seeking an external assessment and documentation for gender-affirming medical care. Contact us for details and support.

  • No Show fee (see practice policy for definition, cancelations are required 24 hours in advance) – The full price of the scheduled appointment type will be charged to your credit card on file.

  • Paperwork completion outside of an appointment (e.g., FEMLA forms, school forms, etc.) Charged in 15-minute increments at $50 per 15 minutes.

For cash pay appointments, payment is expected at the start of each appointment. We can provide you with a “superbill” with codes and data to submit to your insurance if your plan allows for out-of-network care. (Note: We do not control their reimbursement rules and decisions.)

We accept payment by Visa or Mastercard credit/debit cards or FSA/HSA cards (with a Visa or Mastercard logo), ACH check, and Venmo.
